The presentation will provide specific guidelines on how to organize a program to deliver care to a stroke patient, considering all the needs that a stroke survivor and his support system require.  An overview of programmatic features including evidence-based care and patient/family education along with outcome measures will be discussed and elaborated on to provide the participant with tools to implement in his individual stroke program.
