If a course provider has not applied to the FPTA for course approval, you may request individual approval by completing and returning the Individual Application Form.

If you submit this form prior to or within sixty days after course completion, the cost is $20. If submitted later than sixty days after attendance, the cost is $100. If you do not submit all of the required information, there may be additional costs.

2019 Course Pricing Update

Effective immediately, pricing for individual course approval for courses that are being held in 2019 will be as follows:

  • 1-9 hours = $30
  • 10 or more hours = $50

This will apply to applications submitted prior to or within sixty days after attendance and only applies to courses being held in 2019 and beyond, even if approval is being applied for in 2018. All courses submitted more than sixty days after attendance will continue to cost $100.

Pricing for 2018 courses will remain unchanged.
