IMPORTANT – Renew Your License before the Deadline

It is time to renew your physical therapist or physical therapist assistant license. You have until midnight, November 30, 2017 to submit your renewal application to the Department of Health.

Avoid unnecessary delays in the renewal process by reporting your continuing education (CE) hours into the Department of Health's CE tracking system at prior to renewing your license. Your CE records must be complete in order to renew your license.

The Department has a new online services portal. You must complete a one-time account registration process in the new system if you have not done so. This will have to be complete before you will be able to renew your license.

If you have already created your new online services account, simply visit and select “Account Login” from the top right to login to your account.

If you have not yet created your new online services account, following these easy steps:

  1. Visit, select Account Login from the top right.
  2. Select “No” under “Have You Registered in Our New Online Services System?” and follow the instructions provided under “Register.” (Please note your previous user ID and password mailed with your physical license will no longer work with our new system. You must register for a new account using a valid, active email address.)
  3. Follow the prompts to create your new account. Please make sure you add to your trusted/allowed email address list to ensure the temporary password email does not get sent to your spam folder or get filtered out of your email before it is received. Upon registration, check your email for your new account user ID and temporary password. Using your temporary password and user ID provided in the email, log into your account. Enter the temporary password in the Old/Temporary Password field. You will be prompted to create a new password for your account. PLEASE NOTE: The password is case sensitive.
  4. After you sign In with your new user ID and password, you will then be prompted to add your license to your account. You will need three (3) items:
  • Your social security number (*This information is not stored and is for verification and security purposes only)
  • Your date of birth
  • Your current mailing address zip code on file with the Department of Health (Please note that your mailing address zip code can be found on your renewal postcard)

Once you have successfully added your license to your account, from your dashboard page you will see Renew My License section where you will be able to proceed with your license renewal. Your profession will open for renewal 90 days prior to your expiration date. Also, under the Manage My License section or Manage My Application section on your dashboard you will have the option to:

  • add additional licenses or applications
  • request a name or status change
  • update your address and add a secondary practice location
  • upload your documents
  • complete an application you have already started
  • renew your license
