This voluntary program is designed for physical therapists who have completed the APTA CCIP Level 1, and so are APTA-Credentialed Clinical Instructors (CIs), and have supervised and mentored at least one student.

Registration

You will need to submit a completed CCIP Participant Dossier by Thursday, June 7th, 2018. Download the Adobe Acrobat version or Microsoft Word version. **Please note, all dossiers must be typed, no handwritten applications will be accepted. Handwritten applications will be returned for resubmission.**

It is recommended that you download and refer to the Registration Checklist to ensure you completed all of the requirements for registration. Participants must attend all sessions.

Please note there are no refunds and no late registration.

You are required to attend all sessions on both days of the program.
